Opioid addiction : mechanism


The brain has billions of nerve cells, which are also found throughout the whole body. These nerve cells forming a network system called nervous system. Central nervous system is located in the brain while peripheral nervous system located in the other part of the body. This network system function in signal transmission within the body. For example, once a signal is produced in part of the brain, the signal will be transmitted via the network from that part of the brain to the targeted body parts to be activated. This transmission has been made possible by the presence of chemical messengers called neurotransmitters. These chemicals are released by a nerve cell to the adjacent nerve cell when a signal is to be transmitted.

Opioids mimic these chemicals messengers, therefore, able to initiate the signal transmission process within the brain. Opioids tend to induce euphoria/feeling of pleasure by stimulating the brain regions that mediate pleasure. In the brain, there is some parts of the nervous system served as reward pathways. Under normal conditions, the pathways control an individual's responses to natural rewards, such as food, sex, and social interactions, and is therefore an important factor of motivation and drive. In simple words, activation of the pathways tells the individual to repeat what he/she just did to get that reward. In the case of opioids, euphoria is the reward. It also tells the memory centers in the brain to pay particular attention to all features of that rewarding experience, so it can be repeated in the future. The opioid activates the reward pathways leading to repeated abuse , hence addiction.

Drug abuse in long term may result in structural changes in the brain leading to behavioural changes and health problems. The brain becomes conditioned to the consumption of opioid so much that his/her main aim in life is only to get it. This will result in compulsive behaviour in which the users are unable to control self and make sound decision. The body also becomes physically dependent on it. When the opioid are taken away, the users often experience withdrawal symptoms such as restless, muscle and bone pain, cold, diarrhea, vomiting etc.. 

Addiction is not solely a chaotic mind that people often get it wrong by thinking that it is due to a lack of will power or due to the moral failing. It is actually a disease arising from the physiological process occurs in the body, which is initiated by the opioid. Therefore, it is important to know that seeking professional and proper treatment is really necessary to cure it. The treatment is very available and recovery is more than possible. Generally, the treatment programme will consist of  detoxification, management of withdrawal and prevention of relapses by medications and behavioural therapies. Good support from family, friends and loved ones are really helpful. Another important point in addiction management is that relapse is not a reason to be despair. It is actually an opportunity to learn what triggers that should be avoided.

Get Vaccination for Traveling

Many travelers regard vaccination as one of those trivial matters during planning a vacation. They tend to put it down as the last line on their checklist, and often deliberately missed it out for there are much more important things to be done so that they can make the most of their holiday. More often, they do not think of it at all. Travelers are at risk of infectious disease during traveling because some of the destinations are home for these infections. By taking vaccination, the risk of contracting some of these nasty infection can be reduced. Therefore, vaccination should be considered together with other crucial necessities for traveling. 
Among the common infection that typically contracted during traveling is hepatitis A. It is caused by a type of virus, which affecting the liver. Affected individuals may get it by consuming shellfish contaminated with raw sewage. They may develop fever, nausea, vomiting, and abdominal pain. Treatment is conservative. It usually last for few days and is self-limiting. 
Influenza is also common among travelers. It is caused by a type of virus affecting the respiratory tract. The symptoms include cough, runny nose, fever and body ache. Sometimes, it can be serious in high risk group that it may lead to fatal especially when it progresses to respiratory complications such as exacerbation of asthma in asthmatic individuals.
Typhoid is common among travelers that travel to countries with poor hygiene and sanitation. It is caused by Salmonella typhi or Salmonella paratyphi bacteria, which can spread throughout the body. One could get it via fecal-oral route, by which one consuming food or drinks contaminated with feces of someone with typhoid. The most prominent symptom is prolonged fever. Other symptoms might also be present including nonspecific symptoms such as chills, weakness, loss of appetite; gastrointestinal symptoms such as constipation or diarrhea.
Ideally, appointment with a doctor should be arranged 4 - 6 weeks before traveling, as some vaccines require time to take effect in our body. A doctor may need to considered few factors before determine which vaccinations you require. These include age, medical condition, previous immunization, history of developing adverse reaction, and trip destinations.
Infection may be treatable even though some of them may cause death. However, one may have their vacation ruined by getting those infection.Despite enjoying the relaxing and comforting atmosphere in the places of interests, one might have to be admitted to hospital, or may be just staying in the hotel room, lying in the bed for two to three days, due to the infection. Therefore, prevention is still better than cure. Keep the risk of infection to a minimum, so that you can make the most of your vacation.

Introduction
Rheumatoid arthritis is a chronic inflammatory disease of the joints. It typically affects multiple of small joints such as fingers, wrists, toes and ankles, usually in symmetrical manners ,bilaterally. It is also classified under autoimmune disease as the inflammation occur is caused by the autoimmune process, in which there is presence of autoantibodies that attack normal cells and tissues in the body, in this disease it mainly attack the joint tissues. Women seems to be affected more than men.

How it happen?
Immune system in our body is a system that function to protect our body from foreign invaders that may impose harm on the body. This system consists of specialized organs, such as thymus bone marrow, spleen, that produce/store specialized cells including B cells,T cells, APCs, monocytes, plasma cells, etc. These cells work by identifying foreign invaders, initiating a complex cascade, causing inflammation and produce antibodies to attack and destroy the foreign invaders. 
However, in rheumatoid arthritis and other autoimmune diseases, the specialized cells fail to differentiate between the normal tissues and the invaders, they even recognized the normal tissues as the invaders, therefore causing inflammation and produce antibodies to attack them endlessly. The antibodies that attack normal tissues are called 'autoantibodies', which is specifically known as rheumatoid factors in case of rheumatoid arthritis. In rheumatoid arthritis, this process mainly occurs in the joints. It may occurs in other sites, such as heart, kidney , bone marrow, or skin.
The cause is unclear, however, genetic has been suggested to have association with rheumatoid arthritis.

Signs and symptoms
Inflammation in the joints lead to pain, swelling, stiffness and redness. If this condition left untreated, it may progressively lead to damage of the joints causing permanent deformity and loss of function.

Diagnosis
There are combination of criteria used by doctors to diagnosed rheumatoid arthritis. These includes: 
1)Presence of multiple joints inflammation, symmetrical bilaterally, involving the small joints of hands and feet, persist for at least 6 weeks. 2)Subcutaneous nodules, typically over the elbows/fingers. 3)X-ray changes of the joints - periarticular erosions.

Detection of rheumatoid factors alone in the absence of above criteria is not sufficient to make the diagnosis. Actually, the main value of rheumatoid factor test is to determine the severity of the disease. Detection of rheumatoid factor in a persistently high concentration indicate the disease is severe. Not all patients with rheumatoid arthritis have rheumatoid factor. There are some patients whose their rheumatoid factor test are negative, this condition is called seronegative rheumatoid arthritis.

Management
NSAIDs helps especially in the early stage, when pain and stiffnes is the most highlighted issue for the patient at that moment. If NSAIDs is not adequate to control the pain ,corticosteroid usually is very useful as it greatly reduce the inflammation. The most important drugs in the management of rheumatoid arthritis is DMARDs such as methotrexate, as it delays the progression of the disease, however, it can not reverse what has been destroyed. Therefore, the earlier the treatment start, the more benefits patients can get. 

Physiotherapy helps to keep the joints move, which is very important, as unmove joints may lead to contracture deformity. Occupational therapy and mechanical aids help patient especially those with permanent deformity (late stage) to overcome the effects of disability by improving their ability to perform basic tasks in their daily living. Surgery might be indicated for those with deformity if much function is desired.

Closure/tips on rheumatoid arthritis
Although there is no cure for rheumatoid arthritis, but, early detection and early proper treatment is important, for example early usage of DMARDs is more beneficial as the progression of the disease can be delayed from the beginning. Other than that, self-management strategies is very helpful. Patients who learns the techniques and skills to manage their arthritis on day to day basis, including pain management, activities limitation, etc, are more confident and happy in their lives.
