Fibroids may cause abnormal vaginal bleeding, pelvic pain and pressure, repeated miscarriage, constipation, and an urge to urinate more frequently and urgently. These symptoms can be different from one person to another depending on the location of the fibroid.

Doctors may suspects fibroid based on the result of pelvic examination. However, imaging tests such as ultrasonography are usually needed to confirm the diagnosis. In certain cases in which the diagnosis can not be confirm by ultrasonography, other investigations might have to be done in order to confirm the diagnosis, for example sonohysterography or MRI.

Treatment should be individualized, it will depend on either it is symptomatic or not, either the patient desires fertility or not, and either patient has menopause or not. Treatment options may include medications, surgeries and other procedures.

For asymptomatic patient there is no treatment needed. However patient is re-evaluated every 6 - 12 months.

For symptomatic patient, doctors may prescribed medications to control the symptoms. These medications includes hormonal therapy to control the size of fibroid, analgesia to control pain, and antifibrinolytic to control heavy bleeding.

Surgery is only considered when there is indication. Indications for surgery include 1)Rapid enlarging fibroid 2)Recurrent uterine bleeding refractory to medications 3)Severe or persistent or unbearable pain or pressure 3)A large uterus causing compression to other organs and causing dysfunction (hydronephrosis,dyspareunia,urinary frequency, constipation) 4)Infertility (if pregnancy is desired) - myomectomy (removal of fibroid) 5)Recurrent spontaneous abortions 6)Completion of childbearing and patient want definitive management - hysterectomy (removal of uterus).

There are 2 options of surgery :1)Myomectomy (removal of fibroid) 2)Hysterectomy(removal of uterus). If patient desire pregnancy or want to keep the uterus, myomectomy is the choice. If a woman with infertility due to fibroid alone, myomectomy may solve the problem. However, hysterectomy is the most definitive treatment, which involves removal of the whole uterus. After myomectomy, new fibroid may begin to grow again. Therefore, it is not uncommon for patient who have a myomectomy, may choose to have a hysterectomy few years later, when she has complete her family and does not desire pregnancy anymore. When hysterectomy is done, the fibroid will never grow back again. Women that undergone hysterectomy won't be able to conceive, and may have early menopause.

Another procedure for management of fibroids is uterine artery embolization. Using this procedure, unlike myomectomy and hysterectomy in which the aim is to remove the fibroids, the aim of uterine artery embolization is to cause damage to the fibroids, making it shrinks, by blocking the blood supply that feed them , while preserving normal uterine tissues. After this procedure, women recover more quickly than after hysterectomy or myomectomy, but rates of complications and return visits tend to be higher.A woman who desires pregnancy is not suitable for this procedure. It may cause infertility and early menopause.

Tips on uterine fibroids
1.Choose organic food - fibroid is hormone-dependent, estrogen hormone stimulates its growth. Commercial meats may contain estrogen hormone which has been injected to promote growth.
2.Eat fibers - fiber helps to get rid of estrogen.
3.Avoid fried food - estrogen is metabolized by the liver, fried food can increase the load onto the liver, thereby reduce its capacity to process estrogen.
4.Avoid caffeine - caffeine increases estrogen level in the blood
5.Lose weight - As estrogen is metabolized in fat cells, overweight women may have more estrogen compared to women with normal weight
6.Exclude red meat in your diet - red meat increases estrogen level in blood
7.Exercise - it helps in reducing estrogen level in the body
8.Control your stress - stress can induce hormonal imbalances and may worsen the condition. There are a lot of techniques that you can used to help you in managing your stress. For example guided imagery therapy. It is a cognitive - behavioural therapy in which a person is guided in imagining a relaxing scene or series of experiences. Close your eyes for 5 minutes each day and sit in relaxing posture. Imaging and visualize your body being completely free from fibroids. Visualize that all the symptoms have disappeared and you are healthy. Try to repeat this exercise at the same time everyday.
